Ohio has begun distributing COVID-19 vaccines statewide to those who choose to be vaccinated. As COVID-19 vaccines are granted Emergency Use Authorization (EUA) by the Food and Drug Administration (FDA), the state will begin to strategically and thoughtfully distribute the vaccines to Ohioans at the greatest risk in conjunction with recommendations from the Centers for Disease Control (CDC) Advisory Committee on Immunization Practices (ACIP) and the National Academies of Sciences, Engineering, and Medicine (NASEM).
Supply of the COVID-19 vaccine is very limited at this time. While we expect to continue to receive more vaccine as supply increases, we do not have enough supply at this time to meet demand. As vaccine delivery amounts are confirmed, and as Phases are added, the Health Department will open additional appointments and will announce that information here and on the Massillon City Health Department Facebook page. Please click on the phase button that you qualify below for information on how to register for a vaccine.
For additional information visit the Health Department’s web page. You can also reach us by telephone at (330)830-1710, and we are located at 111 Tremont Ave. SW 44647.
Prior to coming to your COVID-19 vaccine appointment you will need to complete the Immunization Adult Health History-COVID form. If you are unable to print a form one will be provided upon your arrival.
