Birth & Death Records
This division can provide certified copies of birth certificates from anywhere in Ohio and death certificates of those who passed away within the Massillon City limits, from 1909 to the present.
For BIRTH AND DEATH CERTIFICATES, print and complete the Application for Certified Copies and mail it with your payment to the Massillon City Health Department. The cost is $25.00 per certificate.
In-person: We accept cash, personal check with valid id, and credit cards (Visa, MasterCard, and Discover). There is a small additional fee for credit card payments.
By mail: Accepted payments include money order, bank check or personal check with a copy of your valid id. Please make checks payable to Massillon City Health Department.

For your convenience, you can request birth and death certificates online at www.vitalchek.com. There is a service fee attached and they accept all major credit cards. Certificates ordered this way are processed the day it is received, or the next business day. Please note that on the Shipping section, United Parcel is the delivery method checked. This is an extra fee for faster delivery. You may change that selection to regular mail if you like. Vitalchek can be used to order certificates from most parts of the country.
We do not have death records for deaths that occurred at Doctors Hospital, nursing homes, and hospitals located outside the Massillon city limits. Please contact Stark County Health Department at 330-493-9904 for further information. Stark county records may also be ordered online at www.vitalchek.com. In the Event Location section, put the township name (Perry, Jackson, etc.) instead of Massillon, so the request goes to the proper health department for processing.

For genealogy purposes, it is still advisable to go to the health department in the jurisdiction where the birth occurred, and you may look at the record. Death records are filed in the health department of the jurisdiction where the death occurred.
